Obtaining a PennCard - Associates

Adjunct Faculty
Adjunct Faculty of the University of Pennsylvania are entitled to a PennCard. Please use the following procedure to obtain your Adjunct Faculty PennCard:

  • Bring your letter of appointment, a completed PennCard Request Form, and a valid form of photo Id (driver's license, passport, etc.) to the PennCard Center.
  • You will then be issued a PennCard good until the date specified on the PennCard Request Form (not to exceed 1 year).
  • There is a $25.00 fee for the card and this charge may be paid by the cardholder by cash, check, Visa or Master Card, or the PennCard Center will bill the cardholder's department as long as the proper 26-digit budget code is entered on the PennCard Request Form.
Courtesy Appointment
Individuals affiliated with a particular University of Pennsylvania department may obtain a Courtesy Appointment PennCard through the sponsorship of that department. Please use the following procedure to obtain your Courtesy Appointment PennCard:
  • Bring a completed PennCard Request Form, along with a valid form of photo Id (driver's license, passport, etc.) to the PennCard Center.
  • You will then be issued a PennCard good until the date specified on the PennCard Request Form (not to exceed 1 year).
  • There is a $25.00 fee for the card and this charge may be paid by the cardholder by cash, check, Visa or Master Card, or the PennCard Center will bill the cardholder's department as long as the proper 26-digit budget code is entered on the PennCard Request Form.
College House Associates
Individuals affiliated with the University of Pennsylvania's College House System may obtain a PennCard by permission of the Office of College Houses and Academic Services. Please use the following procedure to obtain your College House Associate PennCard:
  • Bring a completed PennCard Request Form (this form must be completed by the Office of College Houses and Academic Services), along with a valid form of photo Id (driver's license, passport, etc.) to the PennCard Center.
  • You will then be issued a PennCard good until the date specified on the PennCard Request Form (not to exceed 1 year).
  • There is a $25.00 fee for the card and this charge may be paid by the cardholder by cash, check, Visa or Master Card, or the PennCard Center will bill the cardholder's department as long as the proper 26-digit budget code is entered on the PennCard Request Form.
Chaplain's Office
Individuals affiliated with the Chaplain's Office at the University of Pennsylvania may obtain a PennCard by permission of the Chaplain. Please use the following procedure to obtain your Chaplain's Office PennCard:
  • Bring a completed PennCard Request Form (this form must be completed by the Chaplain's Office), along with a valid form of photo Id (driver's license, passport, etc.) to the PennCard Center.
  • You will then be issued a PennCard good until the date specified on the PennCard Request Form (not to exceed 1 year).
  • There is a $25.00 fee for the card and this charge may be paid by the cardholder by cash, check, Visa or Master Card, or the PennCard Center will bill the cardholder's department as long as the proper 26-digit budget code is entered on the PennCard Request Form.
Howard Hughes Associates
Members of the Howard Hughes Medical Institute are entitled to a PennCard. Please use the following procedure to obtain your Howard Hughes PennCard:
  • Bring a completed PennCard Request Form (this form must be completed by the Howard Hughes Medical Institute), along with a valid form of photo Id (driver's license, passport, etc.) to the PennCard Center.
  • You will then be issued a PennCard good until the date specified on the PennCard Request Form (not to exceed 1 year).
  • There is a $25.00 fee for the card and this charge may be paid by the cardholder by cash, check, Visa or Master Card.
NROTC Faculty
NROTC Faculty at the University of Pennsylvania are entitled to a PennCard. Please use the following procedure to obtain your NROTC Faculty PennCard:
  • Bring a completed PennCard Request Form (this form must be completed by the NROTC or Naval Science Department), along with a valid form of photo Id (driver's license, passport, etc.) to the PennCard Center.
  • You will then be issued a PennCard good until the date specified on the PennCard Request Form (not to exceed 1 year).
  • There is a $25.00 fee for the card and this charge may be paid by the cardholder by cash, check, Visa or Master Card, or the PennCard Center will bill the cardholder's department as long as the proper 26-digit budget code is entered on the PennCard Request Form.
Senior Associates
Senior Associates officially enrolled and registered in a course may obtain a PennCard free of charge by visiting the PennCard Center and presenting a valid form of photo identification (driver's license, passport, etc.). If you are not officially enrolled in a course, you must contact the College of General Studies in order to obtain a PennCard.

Visiting Faculty
Faculty visiting the University of Pennsylvania may obtain a PennCard through the sponsorship of the University department. Please use the following procedure to obtain your Visiting Faculty PennCard:
  • Bring your letter of invitation, a completed PennCard Request Form and a valid form of photo Id (driver's license, passport, etc.) to the PennCard Center.
  • You will then be issued a PennCard good until the date specified on the PennCard Request Form (not to exceed 1 year).

Visiting Scholars
Scholars visiting the University of Pennsylvania may obtain a PennCard through the sponsorship of the University department. Please use the following procedure to obtain your Visiting Scholar PennCard:
  • Bring your letter of invitation, a completed PennCard Request Form and a valid form of photo Id (driver's license, passport, etc.) to the PennCard Center.
  • You will then be issued a PennCard good until the date specified on the PennCard Request Form (not to exceed 1 year).
  • There is a $25.00 fee for the card and this charge may be paid by the cardholder by cash, check, Visa or Master Card, or the PennCard Center will bill the cardholder's department as long as the proper 26-digit budget code is entered on the PennCard Request Form.
Visiting Students
Students Visiting the University of Pennsylvania may obtain a PennCard through the Sponsorship of a University Department. Please use the following procedure to obtain your Visiting Student PennCard:
  • Bring a completed PennCard Request Form and your student ID from the college or university you are currently attending.
  • You will then be issued a PennCard good until the date specified on the PennCard Request Form (not to exceed 1 year).
  • There is a $25.00 fee for the card and this charge may be paid by the cardholder by cash, check, Visa or Master Card, or the PennCard Center will bill the cardholder's department as long as the proper 26-digit budget code is entered on the PennCard Request Form.
Wistar Faculty
Faculty of the Wistar Institute are entitled to a PennCard. Please use the following procedure to obtain your Wistar Faculty PennCard:
  • Bring a completed PennCard Request Form (this form must be embossed with the Wistar seal), along with a valid form of photo Id (driver's license, passport, etc.) to the PennCard Center.
  • There is a $25.00 fee for the card and this charge may be paid by the cardholder by cash or check.
Wistar Staff
Staff of the Wistar Institute are entitled to a PennCard. Please use the following procedure to obtain your Wistar Staff PennCard:
  • Bring a completed PennCard Request Form (this form must be embossed with the Wistar seal), along with a valid form of photo Id (driver's license, passport, etc.) to the PennCard Center.
  • There is a $25.00 fee for the card and this charge may be paid by the cardholder by cash, check, Visa or Master Card.
